Now, you can get your test results on-line or by phone.  If you recently had lab, radiology, or other tests performed at Charlottesville Family Medicine, you were given a wallet-sized card with access information and directions along with a personal identification number (PIN) to obtain your results.

DO NOT THROW THIS CARD AWAY!!  This card will allow you convenient secure access to your confidential test results via the phone or Internet at your convenience through our automated TELEVOX system.  These tests include:

• All lab tests
• X-rays
• Pap exams
• Mammograms
• Ultrasounds
• CT
• MRI
• Skin tests
• And many other lab and radiology tests

With this system, you don’t have to wait for your doctor’s office to call or send you a card.  You can get your results when it is convenient for you (please allow 3-7 business days for your results to be posted, 21 days for Pap test).  Also, if you obtain your test results on-line, you can store them in a personal folder and keep your personal health information in one convenient place.

• To retrieve your test results by phone, call 1.866.934.7105.  Your unique ID number is your chart number provided during your visit (without the letter); your PIN is your date of birth.
• To retrieve your test results on-line, go to www.mytestresults.com.  Be sure you have your wallet-size card with instructions and your medical record (chart) number found on the label at the bottom of the card.

If you would like notification by email, you may sign-up for it when accessing your results on-line.
